The honest summary
Where it may help
There is very little. In one small trial in Varanasi, 50 adults with mildly raised TSH (subclinical hypothyroidism) took ashwagandha root extract or a placebo for eight weeks. TSH, T3 and T4 improved more with ashwagandha. A second trial, in 46 people whose levels stayed off target on levothyroxine, tested an Ayurveda programme added to their tablets.[2][3]
Where evidence is thin
A 2023 review of herbal medicines for hypothyroidism found only four randomised trials, with 186 people in all. The ashwagandha trial was a short pilot in mild disease, and it tells us nothing about taking ashwagandha with levothyroxine. The add-on trial was not blinded, and its abstract gives no sizes of effect.[1][2][3]
When to see an allopath
Standard treatment is a daily levothyroxine tablet, usually for life, with blood tests to keep the dose right. Do not stop levothyroxine to try an Ayurvedic treatment. Not taking prescribed thyroid hormone regularly or correctly is a common cause of myxoedema coma, a life-threatening emergency. Tell your doctor about anything you add.[4][5]
How the texts understand it
Sushruta describes galaganda: a swelling fixed at the neck that hangs like a scrotal sac, large or small. It is a description of a lump in the neck, not of how the thyroid works.[6]
The match with hypothyroidism is loose. In many people with an underactive thyroid the gland is normal-sized or even smaller, so galaganda cannot be used as another name for it. We found no classical term that matches an underactive thyroid itself.[5]
Codes: ICD-11 5A00 (Hypothyroidism)[7]
What trials found
Systematic reviewSystematic review of randomised trials of herbal medicines for hypothyroidism, not only Ayurvedic ones. It found just four trials, with 186 people in all, which shows how little has been tested.[1]
RCTFifty adults aged 18 to 50 in Varanasi, with TSH between 4.5 and 10, took 600 mg a day of ashwagandha root extract or a starch placebo for eight weeks. TSH, T3 and T4 improved significantly more with ashwagandha. Four people, two from each group, withdrew. It was a small pilot in mild disease, and the abstract does not give the size of the changes.[2]
RCTForty-six adults aged 20 to 60 whose thyroid levels stayed off target on a steady dose of levothyroxine, and who stayed on it. For 60 days one group added Kanchanara guggulu and the other a whole-system Ayurveda programme. The abstract reports differences between the groups in TSH, weight and quality of life, but not their size. The authors list the lack of blinding as a limit.[3]
Signs that need a doctor
- Very low body temperature, slowed breathing, low blood pressure, or not responding. These are signs of myxoedema coma. Go to a hospital emergency department.[5]
- Pregnancy, or planning one, when you have an underactive thyroid. Tell your doctor promptly, so your thyroid tests and levothyroxine can be reviewed.[4]
- Symptoms you think could be an underactive thyroid. Many of them have other causes too, so get them checked by a doctor.[4]

Questions people ask
Can thyroid be cured by Ayurveda?
No trial we found has shown that. The trials measured thyroid blood tests over eight weeks or 60 days, and none of them tested Ayurveda in place of levothyroxine.[2][3]
Can I take Ayurvedic medicine with Thyronorm?
Tell your prescribing doctor first. Ashwagandha on its own changed thyroid tests in a trial, so adding it, or a product that contains it, may mean your TSH needs rechecking.[2]
Related
Ashwagandha and thyroid medicines
Ashwagandha can raise thyroid hormone levels. That could add to levothyroxine, or work against antithyroid drugs.
EvidenceDoes ashwagandha change thyroid blood tests?
In one small trial of people with a mildly raised TSH, eight weeks of ashwagandha root extract moved thyroid tests towards normal.
InteractionGuggulu and thyroid medicine (levothyroxine)
Guggul raised thyroid hormone in mice. The one human trial in people on levothyroxine used a multi-herb tablet and cannot show guggul's own effect.
FormulationKanchanar Guggulu
Kanchanar bark, triphala, trikatu and guggulu resin. Listed for lumps and swollen glands; one trial used it in people on levothyroxine.
