Herb and medicine interaction

Berberine and statins

Berberine (found in daruharidra and other plants) (Berberis aristata, one source of berberine) with statins (simvastatin, atorvastatin, rosuvastatin).

Conflicting evidencePending medical review

The short answer

The evidence points two ways. In healthy men, two weeks of isolated berberine raised levels of midazolam, a test drug that shows how active the liver enzyme CYP3A4 is, by about 40%. The simvastatin label forbids its use with strong CYP3A4 blockers. Both human studies gave isolated berberine tablets, not daruharidra (daru haldi) or a classical medicine.

A direct test with simvastatin found no clear change in its levels, but it lasted seven days, in groups of 12 healthy adults. Atorvastatin and rosuvastatin have not been tested with berberine. If you take a statin, tell your prescribing doctor before you start berberine, and report any new muscle pain or weakness.

Simvastatin is sold in India as Simvotin, atorvastatin as Atorva and rosuvastatin as Rosuvas.

At a glance

What was seenHow it may workDirectionEvidence
Midazolam exposure up 40% after two weeks of isolated berberine[1][3]Slows CYP3A4; also slowed CYP2D6 and CYP2C9Could raise simvastatin levels; statins not measuredSmall clinical study
No clear change in simvastatin levels over seven days (berberine chloride)[2]None detectedNo obvious change in simvastatin exposure in healthy adultsSmall clinical study

No listing is not a clearance. A missing interaction here never means a combination is safe.

The evidence, study by study

Crossover study in healthy men, China, 2012 Small clinical study

Eighteen healthy men took isolated berberine, 300 mg three times a day, for two weeks, and a placebo in a separate period; 17 finished. Berberine raised midazolam exposure (AUC) by 40% and its peak level by 38%, and cut its clearance by 27%. Other test drugs showed slower CYP2D6 and CYP2C9 as well. No statin was given, and muscle problems were not studied.[1]

Trial in healthy volunteers, China, 2019 Small clinical study

Sixty healthy adults were split into five groups of 12 and given berberine chloride, simvastatin, fenofibrate or combinations, first as single doses and then for seven days. Berberine caused no clear change in simvastatin levels, and side effects were no more common. The study was open-label, short and in healthy people, so it cannot speak for months of use in patients.[2]

What your doctor may watch for

  • New, unexplained muscle pain, tenderness or weakness, or dark urine.[3]
  • Your doctor may check CK (a muscle enzyme) or liver tests if such symptoms appear.[3]

What to tell your prescribing doctor

You can read this out, or show them this page:

“I am taking, or planning to take, berberine (name of product, how much berberine it contains, dose) along with my statin. Berberine slowed the enzyme CYP3A4 in healthy men, although a short study found no clear change in simvastatin levels. Is this combination a concern for my statin, and what should I watch for?”

Questions people ask

Can I take berberine with atorvastatin or rosuvastatin?

Neither has been tested with berberine. The only direct study used simvastatin, and results for one statin do not carry over to another, so ask your prescribing doctor.[2]

Is daruharidra the same as berberine?

No. Berberine is one compound found in daruharidra (Berberis aristata, also called Indian barberry or tree turmeric) and in plants such as barberry, goldthread and goldenseal. How much berberine a daruharidra powder or classical medicine gives is not known from these studies.[4]
