Herb and medicine interaction

Triphala and blood pressure medicines

Triphala (three-fruit churna) (Emblica officinalis, Terminalia bellerica, Terminalia chebula) with blood pressure medicines such as amlodipine.

Theoretical riskPending medical review

The short answer

Not known in people. We found no study of triphala taken with blood pressure medicines. The concern comes from a Thai lab and rat study. Triphala blocked CYP3A4, a liver enzyme that clears amlodipine, and in rats it raised blood levels of midazolam, a drug cleared by the same enzyme, by about 41%.

If the same happened in people, amlodipine levels could rise. Its label says to watch for low blood pressure and swelling (oedema) when amlodipine is taken with drugs that block this enzyme. Not every BP tablet is cleared this way, so the risk depends on the medicine. If you take a BP medicine, tell your prescribing doctor before you start triphala, and mention any dizziness or new swelling.

Blood pressure medicines sold in India include Amlong (amlodipine), Telma (telmisartan) and Cardace (ramipril).

The evidence, study by study

Rat study, Bangkok, 2022 Animal

In the same Thai paper, rats were given 500 mg per kg of triphala by mouth. Their exposure to midazolam rose by about 41% and to phenacetin by about 61%. No blood pressure medicine was tested, and rat results do not tell us the size of any effect in people.[1]

Lab study, Bangkok, 2022 In vitro

Researchers at Mahidol University and Thailand's National Cancer Institute tested a triphala extract on human liver tissue in the lab. It slowed four drug-clearing enzymes, CYP1A2 most and CYP3A4 next, but did not affect P-glycoprotein, a drug pump in the gut. Lab findings like this show a possible route for an interaction. They do not show that one happens in people.[1]

What your doctor may watch for

  • Dizziness, a drop in blood pressure, or new swelling of the legs. The amlodipine label asks for these to be watched when it is combined with drugs that block CYP3A4.[2]

What to tell your prescribing doctor

You can read this out, or show them this page:

“I am taking, or planning to take, triphala (name of product, dose) along with my blood pressure medicine. A lab study found triphala blocks an enzyme that clears some BP medicines, including amlodipine. Should my blood pressure be checked?”

Questions people ask

Can I take triphala with Telma?

No study has tested triphala with telmisartan (Telma). The lab finding concerns drugs cleared by CYP3A4, such as amlodipine, and we have no evidence on telmisartan either way. Your prescribing doctor knows exactly what is in your tablet.[1]

Does triphala lower blood pressure?

None of the studies we reviewed for this page measured triphala's effect on blood pressure, so we cannot say.[1]
