The honest summary
Where it may help
There is one trial, and it tested a product rather than Ayurveda as a whole. In 68 healthy adults with mild to moderate hair loss, a serum of ashwagandha root extract applied to the scalp for 75 days cut shedding and raised hair density more than a placebo serum. Density rose by 7.3 hairs per square centimetre, against 2.8 with placebo.[1]
Where evidence is thin
From a start of about 160 hairs per square centimetre, that gain is about 4.5%. The difference between the groups was smaller than the trial was designed to detect. Ixoreal Biomed, which supplied the extract, is listed in the trial registry as sponsor. We found no trial of Ayurvedic hair oils or tablets, or of the treatments the classical texts describe.[1][2]
When to see an allopath
Treatment depends on the type of hair loss, and a GP can often tell the cause by looking. For male pattern baldness the main treatments are finasteride and minoxidil. Minoxidil is also used for female pattern hair loss. Hair loss that comes with the other symptoms listed on this page can point to an underlying illness.[3][4]
How the texts understand it
Sushruta explains hair loss this way: pitta at the hair roots, joined with vata, makes the hairs fall, and then kapha with blood blocks the roots so that no new hair grows. He calls this indralupta, khalitya or rujya. It is the text's own explanation, not tested biology. The match is loose. The verse groups these names together, and does not separate pattern baldness from patchy hair loss as doctors do today.[6]
Codes: ICD-11 ED70 (Alopecia or hair loss)[7]; ICD-11 TM2 SN71 (Baldness disorder (TM2))[7]; NAMASTE FA-3 (Khalitya (khalati), listed against SN71. A terminology link, not a one-to-one diagnosis.)[8]
What trials found
RCTDouble-blind trial in India. Sixty-eight healthy adults aged 18 to 45 with mild to moderate hair loss applied an ashwagandha root extract serum or a placebo serum to the scalp for 75 days, and 61 completed. Shedding in a 60-second comb test fell more with ashwagandha, and density rose by 7.3 hairs/cm² against 2.8, from about 160 at the start. The gap between the groups, 4.49 hairs/cm², was below the 10.16 the trial was designed to detect, and the reported effect size is implausibly large. The paper gives hair growth in different units in its methods and its tables.[1]
Signs that need a doctor
- Losing hair fast, or at an early age.[4]
- Hair loss in an unusual pattern, or in patches, including the eyebrows or beard.[4]
- Hair loss that is painful or itchy, or a scalp that is red, scaly or looks infected.[4]
- Hair loss with irregular periods, new facial hair or acne.[4]
- Hair loss with feeling cold, tiredness, muscle weakness or weight gain.[4]
- A patch where the scalp looks smooth and shiny, with no visible hair openings, or redness and swelling around the hair roots. These can be signs of scarring hair loss, which needs a dermatologist's assessment.[5]

Questions people ask
Can Ayurveda regrow hair?
In the one trial we found, an ashwagandha serum added a few hairs per square centimetre over 75 days in people with mild to moderate hair loss. It was not tested on scarring hair loss or on every type of baldness.[1]
Can Ayurvedic oil regrow hair?
We found no trial of an Ayurvedic hair oil. The serum in the one trial is a modern extract product, so its result does not carry over to oils.[1]

One small trial found ashwagandha improved mildly raised TSH. No trial we found tested Ayurveda in place of levothyroxine. Do not stop it.
